ChroNex Care is building a structured way to sustain chronic disease management to facilitate and empower each general practitioner for their patients.
The need
Chronic disease care is a tidal wave to our health care system
General practice is uniquely suited to managing this but we need better support
I am exploring a way to support the delivery of chronic disease management in Australian primary care through strengthening the patient-general practitioner relationship and help the GP to deliver a higher quality chronic disease care for their patients across the course of their therapeutic relationship. General practitioners do this through chronic disease care plans, standard and long appointments and health assessments. However, patients fall through the cracks in the system. Individuals with a high chronic disease burden and complex health needs can have a lower self-management ability and confidence. As such they need additional support. ChroNex Care aims to provide this support and streamline the delivery of chronic disease management in Australian general practice.
OUr Approach
Three principles that guide every decision
ChroNex Care is being built with primary-care clinicians in mind. The product boundary, the evidence base and the safety posture are set up around this.
GP-anchored, not GP-replaced
We seek to enhance a GP's ability to delivery chronic disease management rather than replace them. CDM is highly individualised as the GP knows their patient the best and can determine what are the management priorities at an individual level. This is not something that a system or organisation can automate or replace.
Evidence-first design
Every intervention is grounded in Australian guidelines and peer-reviewed evidence. Where evidence is uncertain, we say so — and design experiments to close the gap rather than overclaim.
Safety and privacy as defaults
Designed with a conservative posture on clinical safety and patient data. We are talking to Australian GPs and practices first, and we treat those conversations — and the standards they hold us to — as the product's guardrails.

Thank you for taking the time to visit this site and consider participating in improving the delivery of chronic disease management in Australian primary care settings.
I am a general practitioner based in Perth, Western Australia and have a deep passion in preventative and chronic disease care. This developed through my research which was originally directed at understanding what affected the delivery of lifestyle management for metabolic syndrome in rural Western Australia. As I conducted my research, my interests evolved to encompass chronic disease management and realised that the key issues are much the same between metropolitan and rural settings, just different in terms of scale and accessibility. I have worked across multiple departments both rurally and in the metropolitan centres which has given me an appreciation for the variety of health issues patients deal with as well as how the health system tries to manage this. My work as a general practitioner has given me a deeper understanding of this infrastructure and the barriers we face in delivering the exceptional care to our patients that we know we can and should be able to deliver. From this understanding, I have founded ChroNex Care to explore and deliver a way to improve chronic disease care by empowering patients, their general practitioners and the rest of the health care infrastructure in managing the burden of complex and chronic disease management.
