ChroNex Care is building a structured way to sustain chronic disease management to facilitate and empower each general practitioner for their patients.
The need
Chronic disease care and the gap between appointments
General practice is uniquely suited to managing this but we need better support
Six in ten Australians live with at least one long-term health condition, and nearly four in ten live with two or more (AIHW 2022). Chronic conditions account for around 91% of Australia's non-fatal disease burden (AIHW, Australia's Health 2024). General practice carries most of this load — through care planning, reviews, recalls and health assessments — and does it well under real constraints.
But the system is built around appointments, and chronic disease doesn't keep appointments. Between one planned contact and the next, patients manage on their own: medicines, goals, monitoring, motivation. Many do well. Others — often those with the most complex needs and the least confidence to self-manage — quietly drift, and the next review discovers the drift too late. Practices see the results every day: recalls that go unanswered, plans made with care that are never revisited, the same preventable deterioration presenting again.
ChroNex Care is studying this gap: what actually happens to chronic disease care between practice contacts, why current tools and programs don't reliably close it, and what practices would genuinely need — clinically, operationally and financially — for better support to be sustainable rather than another burden.
Before anything is built, we are documenting how this works today from the people who do the work: GPs, practice nurses, practice managers and care coordinators. This is a validation-stage inquiry, not a sales process — there is no product demonstration in these conversations, and nothing to buy. What you tell us determines what, if anything, gets built.

Why your perspective matters
GPs
You author the care plans and carry the clinical accountability. We want to understand where the current system supports you and where it leaves you carrying risk alone.
Practice nurses
You run the recalls, the reviews and the day-to-day of chronic disease care. You see where patients disengage before anyone else does.
Practice managers
You know what any change actually costs a practice: rooms, staff time, billing, software. Nothing sustainable gets designed without that lens.
Care coordinators
You work the gap this research is about. Your view of what coordination looks like when it works — and what breaks it — is evidence nobody else can give.
Thank-you honorarium
As thanks for your time, every participant receives a $100 egift card provided after the interview.
How we confirm participants
So that the findings reflect the people who actually do this work — and so the thank-you honorarium goes to the right hands — we confirm each participant’s role before the conversation. In almost all cases this asks nothing of you:
GPs and nurses are checked against the public Ahpra register
Practice managers and care coordinators are confirmed through their practice or organisation’s public listings
Booking with your work email address makes confirmation immediate
Occasionally we may make a brief call to your practice’s front desk — thirty seconds, nothing more
We never ask you to send documents or identification. Confirmation details are used only for this purpose and are covered by the same privacy commitments as the interview. If we can’t confirm a booking, we’ll email you before the scheduled time rather than cancel it.
Privacy
Conversations are recorded only with your consent, for note-taking accuracy. Findings are de-identified before any use. Your name and practice are never published or shared. We will never ask for patient information. Contact details are used solely to arrange the conversation and share findings, and are deleted on request.

OUr Approach
Three principles that guide every decision
ChroNex Care is being built with primary-care clinicians in mind. The product boundary, the evidence base and the safety posture are set up around this.
GP-anchored, not GP-replaced
We seek to enhance a GP's ability to delivery chronic disease management rather than replace them. CDM is highly individualised as the GP knows their patient the best and can determine what are the management priorities at an individual level. This is not something that a system or organisation can automate or replace.
Evidence-first design
Every intervention is grounded in Australian guidelines and peer-reviewed evidence. Where evidence is uncertain, we say so — and design experiments to close the gap rather than overclaim.
Safety and privacy as defaults
Designed with a conservative posture on clinical safety and patient data. We are talking to Australian GPs and practices first, and we treat those conversations — and the standards they hold us to — as the product's guardrails.
About the founder

Thank you for taking the time to visit this site and consider participating in improving the delivery of chronic disease management in Australian primary care settings.
I am a general practitioner based in Perth, Western Australia and have a deep passion in preventative and chronic disease care. This developed through my research which was originally directed at understanding what affected the delivery of lifestyle management for metabolic syndrome in rural Western Australia. As I conducted my research, my interests evolved to encompass chronic disease management and realised that the key issues are much the same between metropolitan and rural settings, just different in terms of scale and accessibility. I have worked across multiple departments both rurally and in the metropolitan centres which has given me an appreciation for the variety of health issues patients deal with as well as how the health system tries to manage this. My work as a general practitioner has given me a deeper understanding of this infrastructure and the barriers we face in delivering the exceptional care to our patients that we know we can and should be able to deliver. From this understanding, I have founded ChroNex Care to explore and deliver a way to improve chronic disease care by empowering patients, their general practitioners and the rest of the health care infrastructure in managing the burden of complex and chronic disease management.
